I feel like I'm taking crazy pills, because no one else seems to admit that the original arcade version of Commando had more than 8 directional shooting.

To me, it seems obvious that a lot of bullets are NOT just going in the standard 8 directions. It's possible to hit enemies that are "in between" vertical/horizontal and the standard diagonal slope.
Am I the only one who realizes this?.

I'm not sure how it decides to shoot at one of those "half step" angles. Maybe when you change from one direction to a neighboring direction, there's a split second when your shot will go in the direction between them.
